Do not attach documents here. Once we have verified your license we will email you for your certificate of insurance, workers' compensation certificate or exemption, and W-9. Nothing sensitive goes through this form, and we never ask for bank details by email.

Workers' compensation * Construction-trade vendors need coverage from the first employee; non-construction from the fourth. If a subcontractor has no coverage, its workers become the hiring contractor's employees for injury purposes — so we cannot dispatch without one or the other.

Certification

I certify that the license, insurance and pricing information above is accurate and current, and that I am authorized to submit it on behalf of this business. *

I authorize Gulf ProClean to verify my license with the issuing authority and to confirm my insurance coverage with my carrier or agent. *

I will notify Gulf ProClean within five business days if my license lapses or is disciplined, or if my insurance is cancelled or non-renewed. I understand dispatch is paused until coverage is current again. *

I will honor the rates submitted above for the validity period I selected, and will give written notice before changing them.
